GAPP Services
The Georgia Pediatric Program (GAPP) is a Medicaid program that provides services to medically fragile children ages 5-21, who are in need of skilled and unskilled services. There is NO OUT OF POCKET COST to the family.

Services are approved based on the child’s medical needs, and families who are eligible for Medicaid can apply for GAPP at any time.


If your child...

Is under 21 yers old.
-
Need help with everyday activities such as feeding assistance, incontinence care, transfer assistance, and bathing.

Has Medicaid.​​​​
They may qualify for in-home care services.
Our Services
We offer a wide variety of home health care services including, but not limited to personal care/ hygiene, mobility assistance, recuperative care, wound care, catheter care, visiting nurses, insulin injections, medication administration, wheelchair assistance, respiratory treatment, and G-tube care.
Home Care
We provide In-Home Care to anyone who needs it. Some services provided include Personal Care, Transfer Assistance, Meal Preparation, Medication Administration, Light House Cleaning Activities, Care After Surgery etc…

Daily Living Activities
Crafting individualized care plans tailored to the distinct needs of each child and family, and ensuring comfort by assisting with all household tasks and emphasizes the customized nature of the care plans and our commitment be your support.

Early Intervention
Allow us to help your child early on. When you first notice they may need a little assistance.

Childcare Services
Everyone needs a break. Allow one of our caregivers to step in and provide child care services; so you can get the break you need!
No matter if it’s a couple of hours or a couple of days; we’ve got you covered.

Caregiver Match
Now that you have approved the home care plan, we will start matching our care givers and nurses with our client. We have a specific algorithm that helps us in this match making process.
All family caregivers and nurses will have book that they will write all care notes in. We also have a supervisory nurse that makes regular unannounced visits during care hours.
*if for some reason you would like to change a caregiver for your loved one we can match them with someone else*

Individualized Care Plan
Our individualized care plans are specific to each client. Based on our assessment notes and client needs, we come up with a recommended plan of action.
We then have another meeting with our clients and their families to review the individualized care plan. Once we are all on the same page we move forward with the home caregiver match.

Initial Call and Intake
This is the first step in the care process where we gather information as a preliminary assessment. We then schedule an in home meeting with our client and their family.
During the in-home meeting we further assess the needs of our client and come up with an individualized care plan.
