Why These Documents Matter
The Georgia Pediatric Program (GAPP) is a Georgia Medicaid program, administered by the Georgia Department of Community Health (DCH), that provides in-home skilled nursing and personal care support for medically fragile children ages 0-21.
Every GAPP request needs prior authorization, and the state bases its decision on medical necessity documented by your child’s physician. A clear, complete packet helps reviewers understand your child’s daily needs without back and forth. Having your papers in one folder before intake makes the first conversation with our team faster and calmer.

02 / What to Gather
Documents Families Usually Gather
Copies or phone photos are fine for most items. If something is missing, bring what you have and we will help fill the gaps.
01
Coverage and ID
Your child’s Medicaid ID card, or the status of a Katie Beckett application. Any private insurance cards, front and back. A photo ID for the parent or legal guardian.
02
Diagnosis Information
A list of your child’s diagnoses as written by the doctor, plus any recent letters or reports that describe them.
03
Physician Contacts
Name, practice, phone and fax for the primary care doctor and each specialist who follows your child.
04
Current Medications
Every medication with dose, route and schedule, including as needed medicines and feeding formulas.
05
Hospital and Specialist Notes
Recent hospital discharge summaries and the latest notes from specialist visits, such as pulmonology or neurology.
06
Equipment List
Each device your child uses at home, with settings where they apply: ventilator settings, feeding pump rate, oxygen flow, suction and monitors.

Helpful Extras to Include
Some details are not medical records but still shape the care plan. Add these to your folder when they fit your family.
- Your child's school or daycare schedule, since care can often continue at school or daycare
- Therapy schedules, such as speech, physical or occupational therapy
- Durable medical equipment supplier name and phone number
- A short note in your own words about a typical day and night of care

Print Your Checklist
Print this page or save it as a PDF, then check items off as you go. Keep the folder somewhere easy to grab, and bring it to intake. Never delay medical care to gather papers: if your child has a medical emergency, call 911. For questions about medications, devices or settings, talk to your child’s doctor.

FAQ
Questions About GAPP Documents
Do I need every document before I call?
No. Call with whatever you have, even if it is only a Medicaid card and a medication list, and we work through the rest together.
My child does not have Medicaid yet. Can we still start?
Yes. Your child must be Medicaid eligible for GAPP, but many families apply through the Katie Beckett pathway, which looks at the child’s own income and resources. Learn more on our Katie Beckett guide.
We have private insurance. Should I include those cards?
Yes. Children with private insurance can still apply, and Medicaid can act as secondary coverage, so we need both sets of information.
How long does the paperwork stage take?
First call and intake happen in the first week, and paperwork and physician orders take roughly the next two weeks. The state’s review usually takes up to about 30 days after a complete packet, and most families start care about 30-60 days after the first call. We cannot promise a date.
Is my child's information kept private?
Yes. We handle records under HIPAA. You can read how we protect health information in our HIPAA notice.
What if my child's needs change after approval?
Hours are reviewed and re-authorized periodically. If needs change, tell us and share any new discharge summaries or orders so we can request an update.
