This Program Wasn't Designed To Be Marketed. We Exist So Georgia Families Can Find It.

The Georgia Pediatric Program (GAPP) has quietly provided in-home nursing for medically fragile children since 2002. Yet many of the families who need it most have never heard of it. Parents spend years managing feeding pumps, seizure plans, trach care and sleepless nights on their own, believing there is no other option.

SLB In-Home Care was built to change that. We connect Georgia families with the skilled nursing and personal care their children are entitled to, then deliver that care with the warmth of someone who understands what parents are carrying.

We are deliberately focused: children with complex medical needs, cared for where they are most comfortable: at home. Every care plan is built around the child, not around a staffing spreadsheet.

Through GAPP, children can receive two kinds of in-home support. Your child’s doctor and the state determine what is medically necessary; our nurses deliver it, including overnight and respite shifts.

Who We Serve

Children Ages 0-21 With Complex Medical Needs

GAPP is for Georgia children who are medically fragile and need skilled nursing or personal care. Eligibility is based on medical necessity, not family income, and many conditions can qualify, including more than 50 we see regularly.

Neurological

Cerebral palsy, traumatic brain injury, seizure disorders and hydrocephalus.

Genetic

Down syndrome with medical needs, Angelman syndrome, Rett syndrome and rare genetic conditions.

Physical

Muscular dystrophy, spina bifida, paralysis and children who depend on a wheelchair.

Technology-Dependent

Ventilators and trachs, feeding tubes, central lines and other medical equipment.

GAPP is a Georgia Medicaid program administered by the Georgia Department of Community Health. Every service requires prior authorization, requested through an enrolled GAPP agency like ours. Here is how it works for your family, from the first call to the first shift.

Is SLB In-Home Care a GAPP provider?

Yes. SLB In-Home Care is an enrolled provider for GAPP, a Georgia Medicaid program administered by the Georgia Department of Community Health. Families are free to choose any GAPP provider, and the state makes the final eligibility decision. Learn more on our About GAPP page.

What if my child has private insurance?

Your child can still apply. For children who qualify, Georgia Medicaid can act as secondary coverage alongside private insurance.

What happens if our GAPP request is denied?

Denials are not the end of the road. We review the decision with you, gather any additional documentation from your child’s doctor, and help you file an appeal.

What is Katie Beckett, and how does it relate to GAPP?

Katie Beckett is a Georgia Medicaid pathway for children with significant medical needs that looks at the child rather than the parents’ income. Many families become Medicaid eligible through it, which is one of the requirements for GAPP.

Can we change nurses if it is not the right fit?

Yes. Your comfort matters. Tell us what is not working and we will work with you to find a nurse who is a better match for your child and your home.

Can nursing hours change as my child's needs change?

Yes. Nursing hours are approved by the state through prior authorization. When your child’s needs change, we document the change and request an updated authorization.

Is our family's health information kept private?

Yes. We follow HIPAA privacy rules for every family we serve. You can read how we protect your information in our HIPAA Notice.

Can nursing continue at school or while we travel?

In many cases, yes. Nursing support can often go with your child to daycare or school, and we plan coverage around family vacations. See How It Works for details.
