Our Neighbors
A College Park Office on the Clayton County Line
Clayton County families do not need to look far for pediatric home nursing. Our office sits in College Park, where Fulton County meets Clayton, so the trip down to Jonesboro or across to Riverdale is part of our everyday routine rather than a long haul.
That closeness shapes how we work. Coordinators plan shifts with real drive times in mind, and a parent who wants to talk face to face can visit during office hours: Monday to Friday 7am to 7pm, and Saturday and Sunday 11am to 5pm.
Outside those hours, you can still reach us through 24/7 online support by web form or message, and we follow up during office hours. Whichever way you reach out, we can talk through your child’s needs, your insurance situation and what the GAPP process looks like.

What We Provide
Care That Comes to Your Clayton County Home
The Georgia Pediatric Program (GAPP) is a Georgia Medicaid program, administered by the Georgia Department of Community Health (DCH), that provides in home skilled nursing and personal care support for medically fragile children ages 0-21. Because GAPP runs statewide, a Jonesboro family has the same access to it as a family anywhere else in Georgia. Our licensed RNs and LPNs focus on children, and our personal care aides handle the daily routines that keep a household running.

Skilled Nursing (RN and LPN)
- Tracheostomy and ventilator care
- G tube feeding and medication administration
- Seizure monitoring and documentation
- Ongoing nursing assessments
Personal Care Support
- Bathing and dressing
- Help with feeding
- Mobility and toileting
- Supervision during the day
Flexible Scheduling
- Overnight shifts so parents can sleep
- Respite shifts for a needed break
- Care that can often continue at school, daycare or while traveling
How to Begin
Starting Care From Jonesboro, Step by Step
Most families start care about 30 to 60 days after the first call. We cannot promise a date, because the state makes the final decision, but we keep you informed at every stage.
01
Week one
First Call and Intake
Tell us about your child, home and schedule, whether you are coming home from Children’s Healthcare of Atlanta, Southern Regional Medical Center in Riverdale or another hospital. We confirm Medicaid status, including the Katie Beckett pathway, based on the child’s own income and resources.
02
Next two weeks
Orders and Records
We work with your child’s physician to gather the orders and medical records that document medical necessity, so you are not chasing paperwork alone.
03
Up to about 30 days
State Review
We prepare and submit the prior authorization packet. Review usually takes up to about 30 days after a complete packet, and we track it for you.
04
About 30 to 60 days
Matching and First Shift
You help choose your nurse, we build the schedule, and care begins at home. If the request is denied, we help you with the appeal.
In an Emergency
If your child is having trouble breathing, a seizure that will not stop, or any other emergency, call 911 right away. Home nursing does not replace emergency services, and clinical decisions belong with your child’s doctor.
Common Questions
Questions From Clayton County Families
Does SLB In-Home Care serve all of Clayton County?
Clayton County is one of the four counties we serve, along with Fulton, DeKalb and Fayette. For a specific address, call 404-905-8332 and we will check it for you.
What does GAPP cost a Jonesboro family?
There is no out of pocket cost for approved hours when approved by Georgia Medicaid.
We have private insurance. Can our child still apply?
Yes. Children with private insurance can still apply, and Medicaid can act as secondary coverage.
How many nursing hours will my child get?
The state approves hours based on documented need. Hours are reviewed and reauthorized periodically, and you can ask for an update when your child’s needs change. See the official GAPP policy manual from Georgia Medicaid.
We already have a GAPP agency. Can we move to SLB?
Families choose their GAPP provider and may switch agencies. Our guide to switching GAPP providers explains how the handoff works.
What if we do not click with our nurse?
Tell us. Families take part in matching, and you can request a different nurse.
Can I email documents instead of driving to the office?
Yes. Send them to admin@slbinhomecare.org, or text us at 404-905-8332 to ask about the easiest option.
